How does this predict CTR without real data?

It doesn't — it estimates. The score is a heuristic weighted composite of six visual factors that correlate with CTR in published research: contrast, brightness, saturation, warm palette share, proper aspect, and dominant hue. It flags which levers are weak. Real CTR comes from A/B testing.

Why is warm palette weighted so heavily?

Statistically, red / orange / yellow out-click cool palettes on YouTube's suggested feed. It's not the only factor — but if you've got a dominant blue or green thumbnail and your CTR is below 5%, this is the first thing to change.

Doesn't size / resolution just matter for YouTube accepting the upload?

Partly — YouTube requires 1280×720 minimum for monetization. But the bigger reason is that lower-resolution thumbnails get up-scaled by YouTube's player, which softens text and weakens the hook at small feed sizes.
